Have you been invited to participate in the Factor 4 Index survey?
- Your opinion: you can give your opinion anonymously by answering the 64 questions online within the period specified.
- How does it work? The email you have received includes a unique link. You can access the Factor 4 Index website in order to answer the questions by clicking on the link.
There are no right or wrong answers in the Factor 4 Index; we are only asking about your experiences. It is therefore very important to us that you answer the questionnaire as honestly as possible.
- View personal results: your scores are saved in the database once the questionnaire has been completed. You can view and save your scores up until the closing date of the questionnaire. This is done using the personal link at the bottom of the invitation. The scores are divided into 16 categories. You are the only person who can access your individual scores. Your personal link will become invalid once the questionnaire has been closed.

- Should you participate? Of course you are not obligated to participate, but it is an opportunity to share your opinion. But it is important that you participate since your input allows your organization to create appropriate solutions for any possible problems.
- Cookies: the Factor 4 Index uses cookies to easily get participants who have interrupted their session back on track. This means you can exit the website at any time and then use your personal link to continue later on.
- Anonymity: the anonymity of your participation is guaranteed. individual scores cannot be requested. The Factor 4 Index does not provide any information regarding individual answers. You will be asked to provide some personal information before completing the questionnaire. The Factor 4 Index uses this information solely for comparisons between, for example, positions or organizations.
There must be at least 5 participants before the results are made available to the organization for reasons of the anonymity of participants.
